Notes GFR general:
Renal function-replacement therapy
CAPD: Amoxicilline wordt niet uitgescheiden middels CAPD, doseer alsof eGFR< 30 ml/min
Hemo dialysis: Amoxicilline wordt uitgescheiden door Hemodialyse, doseer alsof eGFR< 30 ml/min. Neem een controlespiegel af voor en direct na dialyse. Geef vervolgens na de dialyse een extra gift van 1 gram.
CAV / VVHD: Amoxicilline wordt uitgescheiden door CVVHD, doseer alsof eGFR 30-50 ml/min en controleer de spiegel ca 24 uur na start therapie.
