Renal function-replacement therapy
CAPD: Oplaaddosis 1 g, gevolgd door 0.5-1 g per dag in 1 dosis of als continue infusie
Hemo dialysis: Eenmalig normale aanvangsdosis, gevolgd door 2g-2g-3g op dialysedagen toedienen, na dialysesessie. 2g voorafgaand aan 48 uurs interval en 3g voor 72 uurs interval tot volgende dialyse. Kan op dialyseafdeling worden toegediend, iom consulent interne.*

General comments
*Bij HD patiënten is het advies om de dalspiegel te bepalen om toxiciteit door te hoge blootstelling uit te sluiten. Spiegel dient te worden bepaald na 2 dialysesessies (als dalspiegel vóór dialyse). Streefwaarde dalspiegel: 8-25 mg/L (niet evidence based, afgeleid van een ceftazidim MIC van 8 mg/L; omdat de eiwitbinding van ceftazidim ca. 10% is levert een dalspiegel van >8 mg/L een T>MIC van 100% op).
