General comments

Houd altijd rekening met een mogelijk verhoogd of hoog risico op ESBL-dragerschap.

  • Toevoegen tobramycine bij verhoogd risico op ESBL dragerschap:
    • infectie hospital-acquired/health-care associated
    • recent (<2 maanden) gebruik breed-spectrum antibiotica
    • recent (< 3 maanden) reis naar hoog-endemisch gebied (zie https://resistancemap.cddep.org/)
    • recente (< 2 maanden) ziekenhuisopname in buitenland

 

  • Therapie vervangen door meropenem bij hoog risico op EBSL dragerschap:
    • kolonisatie of infectie met een ESBL in het afgelopen jaar
