Comments: Continue toediening heeft de voorkeur. Bij patiënten met een lichaamsgewicht onder de 60 kg wordt opgeladen met 1000 mg en boven de 60 kg met 1500 mg. Indien continue toediening niet mogelijk is (door lijn-bezetting zoals bij hematologie-patiënten die tegelijk bloedproducten en TPV toegediend krijgen) dan intermitterend. LET OP: bij vancomycine moeten spiegels worden afgenomen (bij continue toediening: binnen 8-20 uur na start, bij intermitterende toediening: top- en mid/dalspiegel). Renal function-replacement therapy

CAPD: geen invloed, 15 mg/kg i.v. elke 4-7 dagen; intraperitoneaal diverse schema’s bijv. 2 g ip 1x/5-7 dagen op geleide van spiegel.

Hemo dialysis: Op dialyseafdeling toegediend aan eind van dialyse. <70 kg: 1000 mg oplaad gevolgd door 500 mg onderhoud. 70-100 kg: 1250 mg oplaad gevolgd door 750 mg onderhoud. >100 kg: 1500 mg oplaad gevolgd door 1000 mg onderhoud. Wordt gedoseerd o.g.v. spiegels.

CAV / VVHD: Normale oplaaddosering conform het iprova document Antibiotica tabel Intensive Care gevolgd door 15 mg/kg/dag per dag als continue infusie (max 3g/24 uur). Spiegelafname na 8-24 uur.
